- (3) 25 minute follow up visits within 90 days
- 1 school or work letter, if recommended

RESTORATION PLAN
- Individuals with history of ADHD seeking for alternative treatment options who may/may not mitigate symptoms with medication
- Individuals with ADHD history who also have comorbid psychiatric symptoms
